Use the public website contact form and ordinary email for general questions and facility partnership inquiries only. Do not include patient names, dates of birth, diagnoses, symptoms, medication information, test results, insurance identifiers, medical records, or other patient health information.

These channels are not intended for clinical records or patient-specific treatment requests. A website privacy statement does not turn a general contact form into a protected clinical intake system. If you need to discuss a patient, request records, or exercise a privacy right, call (904) 776-9126 to arrange an appropriate communication method.

Patient Referrals & Records

For referral coordination and records-transfer instructions, call (904) 776-9126. Our listed fax number is (570) 554-8214. Before sending patient information, confirm the destination, intended recipient, and approved transfer method with our team; use your organization’s approved secure fax process or another method our team confirms.

Send only information appropriate for the referral and permitted by applicable law. Do not send records through the public contact form or ordinary email. A referral inquiry does not guarantee acceptance, scheduling, or establish a provider-patient relationship.

Privacy Questions & Requests

Call and ask for the Privacy Officer to discuss records access, corrections, confidential communications, restrictions, or a privacy complaint.
